The Personal Health Profile (PHP) is a questionnaire that provides employees with a personalized, confidential assessment of their health risks along with recommendations for improving specific health behaviors. The questions cover a broad range of topics including smoking, exercise, nutrition, sleep habits, stress, and other gender-specific health topics.²
The Executive Summary Report
The Executive Summary Report provides you with an analysis of the combined data from your employees’ PHPs. This data will help you create a health promotion program that is appropriate for your worksite by allowing you to:
- design a focused worksite wellness program;
- target health promotion efforts to specific risk areas;
- develop programs that reflect the health needs and interests of your employees;
- establish a baseline for health and health risks within your employee population so you can evaluate the effectiveness of future worksite health promotion activities.
Note: The Executive Summary Report is available only for groups of more than 100 people, and a minimum of 30 percent employee participation is required before an Executive Summary Report can be generated.
