Connections Complex Care Management Program

The Connections Complex Care Management Program supports eligible members with one or more of 16 complex, chronic conditions. The program provides a care management nurse who works with members and their health care team to offer support through education, guidance, and assistance in monitoring their health. A complete list of conditions includes:

• amyotrophic lateral sclerosis (ALS)
• chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)
• Crohn’s disease
• cystic fibrosis
• dermatomyositis
• Gaucher disease
• hemophilia
• multiple sclerosis
• myasthenia gravis
• Parkinson’s disease
• polymyositis
• rheumatoid arthritis
• scleroderma
• seizure disorders
• sickle cell disease
• systemic lupus erythematosus
