Updated Procedures for Requesting Precertification for Pain Management, DME, and Home Health Providers
Effective September 1, 2011, updated procedures will apply to precertification requests for pain management procedures (i.e., paravertebral facet joint, transforaminal epidural, and epidural injections), CPAP and BiPAP machines, and physical, occupational, and speech therapy in the home beyond the initial 60 days.
