New Email Address Replaces the Network Credentialing Support Services Hotline (PA and DE only)
Effective November 1, 2010, the Network Credentialing Support Services Hotline (215-241-4120 for PA and 302-661-6113 for DE) previously designated for requesting your contract and billing registration forms, will be replaced with an email address, Application_Requests@amerihealth.com. Providers interested in participating in our network are responsible for notifying AmeriHealth upon completion of their Council for Affordable Quality Healthcare (CAQH) credentialing form. Once the form is completed, providers should notify the Network Credentialing Support Services Department by using the email indicated above. Please be sure to include the following information:
- practitioner’s name
- practitioner’s mailing address
- practitioner’s office address
- practitioner’s specialty
- practitioner’s CAQH ID number
- contact person’s name, telephone number, and email address
