The 2010-11 Clinical Insights: Effective Care for Patients with Chronic Conditions is now available. Clinical Insights highlight topics that Health Coaches from the Connections^SM Health Management Program may discuss with members who have the following conditions:

• asthma
• permanent (chronic) atrial fibrillation
• chronic obstructive pulmonary disease
• diabetes mellitus
• heart failure
• primary and secondary stroke prevention

Information for each condition is extracted directly from the Clinical Practice Guidelines.
